Long-term follow-up of minimally invasive video-assisted thoracoscopic surgery with epicardial radiofrequency ablation for complex cases of inappropriate sinus tachycardia

چکیده
Introduction Inappropriate sinus tachycardia (IST) is a rare disorder defined as a nonparoxysmal rhythm originating from the sinus node associated with palpitations with a resting heart rate 4100 beats/min and a 24-hour mean heart rate 490 beats/min, with all other potential causes of sinus tachycardia excluded. Patients with IST may experience debilitating palpitations, lightheadedness, chest discomfort, poor exercise tolerance, and other multisystem complaints. It has a complex pathogenesis with many proposed mechanisms, including increased resting sympathetic tone, decreased parasympathetic response, and increased intrinsic sinus node rate. Treatment of IST is variably successful in reducing symptoms. If β-blockers and calcium channel blockers are ineffective, ivabradine, an inhibitor of the hyperpolarizing sodium current, may be better tolerated. For medically refractory symptoms, several methods of endocardial and epicardial ablation to modify the sinus node have been developed. Long-term follow-up is unknown for many of these techniques. We report 2 patients with complex cases of IST and debilitating symptoms despite medical therapy and previous invasive interventions. Both patients were successfully treated with epicardial radiofrequency ablation facilitated by minimally invasive video-assisted thoracoscopic surgery (VATS), with no recurrence of sinus tachycardia after up to 4 years of follow-up.

Late presentation of constrictive pericarditis after limited epicardial ablation for inappropriate sinus tachycardia
The number of radiofrequency catheter ablation (RFA) procedures performed in the epicardial space is increasing. Major acute complications (primarily pericardial bleeding) and delayed complications have been reported in 5% and 2% of cases, respectively. To the best of our knowledge, a single case of constrictive pericarditis after multiple epicardial ablations for ventricular tachycardia has be...
متن کاملMinimally invasive Cox-maze procedure, beating-heart epicardial ablation, hybrid procedure and catheter ablation: a call for comparative evidence.
We read with great interest the paper by Je et al. [1] regarding minimally invasive surgical treatment for atrial fibrillation. This was a comprehensive systematic review pooling available data on the Cox-maze procedure, beating-heart epicardial ablation and hybrid procedure to compare their safety and efficacy. Based on 37 studies included in this review, Je et al. demonstrated that at 12-mont...
متن کاملMinimally invasive surgical implantation of left ventricular epicardial leads for ventricular resynchronization using video-assisted thoracoscopy.
BACKGROUND AND OBJECTIVES Cardiac resynchronization via left ventricular or biventricular pacing is an option for selected patients with ventricular systolic dysfunction and widened QRS complex. Stimulation through a coronary vein is the technique of choice for left ventricular pacing, but this approach results in a failure rate of approximately 8%. We describe our initial experience with minim...
متن کاملMinimally invasive off-pump video-assisted endoscopic surgical pulmonary vein isolation using bipolar radiofrequency ablation - preliminary report.
BACKGROUND Atrial fibrillation (AF) is the most common arrhythmia, having a strong impact on long-term stroke and heart failure prevalence and mortality. Nowadays, rapid development in the field of minimising the invasiveness of surgical ablation of AF can be observed. AIM To report on the feasibility and early results of the first Polish experience with a novel technique of minimally invasiv...
متن کاملAnalysis of the Results of Pulmonary Resection by Minimally Invasive Thoracoscopy for the Surgical Treatment of Lung Cancer
Introduction: Lung cancer is the disease of modern era, and the rate of lung cancer mortality is three times as high as that for prostate cancer and twice as high as the rate for breast cancer. We aimed to analyze the results of pulmonary resection in patients with NSCLC by minimally invasive thoracoscopy. Materials and Methods: We studied 10 patients with NSCLC scheduled for surgical resectio...
